Saint Jude Census Form

Welcome to this site. I hope that you will find this an easy way to complete our new census form which is urgently needed for our parish. Please take your time and relax in your home to complete this form. The instructions are easy to follow. Your information is absolutely necessary to us.

It has been many years since a census has been taken for our parish. It is vital for us to know how many registered members we have in our parish community so that we can develop a budget for the year that will pay the ongoing expenses and at the same time allow planning for needed repairs and expansions.

Since the last census, some parishioners have left, but they have been replaced by new families. At the moment, that is an unknown figure. Hopefully, this new census will bring us to the current status of our parish family. From this figure, we will be able to get a clearer understanding of where we are going with our church programs, how we can pay off our debt to the Diocese, and what direction we need to take for our latest crisis - a new roof for the hall and the church.

Form Instructions

Please read these directions before you click on the link to open up the form.

• The first thing you will do is click the link for the form. This will open up in a new browser window. Please allow some time for it to open up in your browser.
• Once the form is loaded up you'll see that it looks like an actual document where you can print it out and fill out the information. But you shouldn't have to do that here. Each line where you would enter text manually with a pen or pencil, you can type it in on the form itself.
• The first thing you do on the form is fill out the date. It should be in the format of 12/30/2006 where it is month first, then day and finally year.
• The rest of the form should be pretty self-explanatory.
• The phone number sections don't have ( ). When you enter the area code then the phone number and complete it, the (areacode) will show up.
• If you are having problems trying to read the text, you can zoom in with the plus and minus icons at the top of the new window. Or you can click off to the side and hit the Ctrl key with either the plus or minus key on your keyboard. (Not the one on your number pad.)
• Once you are satisfied with the information you have entered, you can then go back to the top of the page. Here you will see a Submit Form button. Click that and the information you entered will be emailed to the person who will be handling it.
